How to install .lrtemplate presets in Lightroom 4, 5, 6 and Classic 7.2 or older

Quick answer
In Lightroom 4–6 or Lightroom Classic 7.2 and older, go to the Develop module, right-click User Presets in the Presets panel, choose Import…, and select the .lrtemplate files from the LRTEMPLATE folder.

Step by step
Unzip your download
Use the LRTEMPLATE folder.
Open the Develop module
Select a photo and press D.
Import
In the Presets panel, right-click User Presets and choose Import…. Select the .lrtemplate files and click Import.
Troubleshooting
I have the newest Lightroom Classic. Should I use these?
No. Current Lightroom Classic doesn't reliably import .lrtemplate files, and Lightroom (desktop) doesn't support them at all. Use the .xmp files and the Lightroom Classic guide instead.
